What is the size of the organizer?
All editions are in standard 8 1/2" x 11" format for ease of use with other systems and for easy filing of handouts and reports from your physicians and associated laboratories.
What is the difference between the Couple and Family Editions?
The Couple Edition is intended for two adults. The Family Edition is intended for two adults and up to three children. If you have more than three children, just let us know and we will make sure you get the right number of forms.
What if I do not go to the doctor very often?
Even if you do not go to the doctor very often, it is good to keep track of your medical history. If a situation arises where you are referred to another doctor or for a diagnostic test, you will need to provide information regarding your medical history. And if you change insurance companies, you may be asked to provide information going back ten years. The sooner you start, the more complete your history will be.
Do I have to buy a complete set (binder, tabs and forms)?
No. Although the complete package of a binder, tabs and forms is recommended for ease of use, each component can be purchased separately. Click Here
What if I do not have any serious medical issues?
First of all, congratulations! However, keeping track of your health information is a good way to be proactive regarding your health. You can use the organizer to keep track of your physical exams, your exercise and diet, and various preventive diagnostic tests.

Can more than one person use one book?
Yes, by using either the Family or Couple Edition. Both versions have a place for the user name on each form and come with multiple illness histories and family health trees. In addition, the Family Edition has both adult and child immunization schedules.

How can I get historical information?
You can try a number of sources. Your pharmacy should be able to give you a list of medications that you have purchased for the past few years. This information can be 3-hole punched and inserted into the What Medications are You Taking? section. It can also be used as a reminder of injuries or illnesses that you have experienced and the time that they occurred. If you keep your check registers or credit card statements, you can also go through those and see when you made payments to hospitals, radiologists, etc. This will help in identifying hospital stays, ER visits and diagnostic tests. You can also ask your insurance company to give you a list of the claims you have filed. And, of course, you can always ask your doctor for information.
