Return Policy


Forms and Tab Sets

Unopened product may be returned for a full refund (excluding shipping costs) within 30 days. However, due to the nature of this product, opened forms and/or tab sets may not be returned.


All Other Products

Unopened product may be returned for a full refund (excluding shipping costs) within 30 days.


Any product with a manufacturer defect may be returned for replacement within 30 days. Shipping costs for return of defective product will be reimbursed at the USPS standard shipping rate.


All returned products must be received in the original condition in which they were shipped and must be accompanied by a return authorization number (RA#). Simply call our toll free number 1-888-TELLME2 (1-888-835-5632, click here to request/obtain your RA# or for any additional return questions you may have.

If you can not find the information you need within our FAQs or policy pages, you may contact us via email, contact form, phone, fax or mail. We are available to answer your phone questions 10:00 am to 5:00 pm CT, Monday through Friday.


Shipping Policy

The total delivery time for your order is based on the processing time and shipment method selected. Orders are processed with 48 hours of receipt, except for weekends and holidays.


Express services are generally not available for PO boxes, FPO and APO addresses or US territories.

International orders are subject to any applicable customs fees, duties or taxes and are the responsibility of the recipient of the order.

We accept American Express, Discover, Master Card, Visa, and PayPal.


Sales Tax

With apologies to Illinois residents, we are located in Cook County, outside of Chicago. Therefore Cook County taxes (currently 9.25%) are applicable to orders shipped within the state of Illinois.
